If you have irregular periods, unexpected breast discharge, or difficulty conceiving, a condition called hyperprolactinemia could be the reason. It is highly treatable, and restoring natural fertility is usually achievable with the right care. At Yaal Fertility Centre in Chennai, Dr. Rukkayal Fathima and the team check prolactin as a routine part of every fertility workup.
What Is Hyperprolactinemia and What Does Prolactin Do?
Prolactin is a hormone made mainly by the pituitary gland. Its main job is to stimulate breast development and milk production after childbirth. Outside pregnancy and breastfeeding, prolactin is kept low by dopamine released from the hypothalamus. If that control is disturbed, or the pituitary overproduces prolactin, the result is hyperprolactinemia: abnormally high prolactin in someone who is not pregnant or nursing.
The Role of the Pituitary Gland
The pituitary sits at the base of the brain and helps coordinate the thyroid, adrenal glands, growth and reproduction. Because it is linked to the hypothalamus, anything that interrupts the dopamine "brake" can raise prolactin.
Normal vs. Elevated Prolactin Levels
Typical laboratory ranges differ between labs. As a rough guide, non-pregnant women are generally under about 25 ng/mL. Prolactin rises naturally in pregnancy. Mild elevations can be due to stress, exertion or minor causes, whereas very high levels (for example over 100–200 ng/mL) can suggest a pituitary growth. Your lab's own range and your doctor's interpretation are what matter.
Common Symptoms of High Prolactin Levels
Some women have no symptoms and find out only during an infertility workup. Others notice clear changes.
Menstrual Irregularities and Amenorrhea
- Oligomenorrhoea: infrequent cycles, longer than 35 days.
- Amenorrhoea: no periods for three months or more.
High prolactin stops the ovaries receiving the signals they need to release an egg. Without ovulation, the cyclical progesterone that builds and sheds the lining is missing, leading to erratic or absent periods.
Understanding Galactorrhea
Galactorrhea is milk-like discharge from the breasts in someone who is not pregnant or breastfeeding. It is a classic sign, but many women with high prolactin never have it, so its absence does not rule out the condition.
Other Systemic Symptoms
Low estrogen from high prolactin can cause vaginal dryness and painful sex, reduced libido, mood changes, and, if untreated for a long time, loss of bone density.
What Causes Hyperprolactinemia?
Hyperprolactinemia is a sign of something else, so treatment depends on finding the cause.
Prolactinomas (Pituitary Adenomas)
A prolactinoma is a benign tumour of the pituitary that makes prolactin.
- Microprolactinoma: under 10 mm. Very common, rarely grows, and usually managed with medication.
- Macroprolactinoma: 10 mm or larger. It may press on nearby structures and cause headaches or visual field changes.
Both types generally respond well to medical treatment.
Medication-Induced Hyperprolactinemia
Drugs that block dopamine can raise prolactin, including some antipsychotics, some antidepressants, certain blood pressure medicines (such as methyldopa and verapamil) and anti-nausea or reflux drugs (such as metoclopramide and domperidone). Never stop a prescribed medicine on your own; speak to your doctor.
The Thyroid-Prolactin Connection
In an underactive thyroid, the brain increases TRH, which stimulates both TSH and prolactin. So treating the thyroid can bring prolactin down. See thyroid and fertility.
Physical and Emotional Stress
Prolactin is sensitive to stress. Temporary rises can follow severe stress, strenuous exercise, poor sleep, breast stimulation or even a recent meal. That is why blood tests are done under resting conditions and repeated if the first result is mildly raised.
How High Prolactin Impacts Female Fertility and Ovulation
The Suppression of GnRH and Ovulation
Normally the hypothalamus releases GnRH in pulses, prompting the pituitary to release FSH (which grows follicles) and LH (which triggers ovulation). Raised prolactin suppresses GnRH pulses, so FSH and LH fall, follicles fail to mature and no egg is released (anovulation).
Luteal Phase Defects and Implantation Issues
In milder cases ovulation may still happen occasionally, but the corpus luteum may produce too little progesterone. The lining then breaks down early, the luteal phase is short, and an embryo may not implant securely, leading to early implantation failure or very early miscarriage.
How Is Hyperprolactinemia Diagnosed?
Serum Prolactin Blood Tests
The main test is a simple blood test. To improve accuracy:
- it is best done in the morning, a couple of hours after waking, ideally fasting;
- rest quietly for 15–20 minutes before the blood draw;
- avoid breast stimulation for 24 hours beforehand.
A mildly raised result is normally repeated to check it wasn't a temporary spike.
MRI Imaging for Pituitary Evaluation
If prolactin stays clearly raised, especially at higher levels, or if there are headaches or visual changes, an MRI of the pituitary is recommended. It shows whether there is a prolactinoma, how big it is, and whether it presses on nearby structures.
Medical Treatments: How to Lower Prolactin Levels
The goals are to normalise prolactin, shrink any tumour and restore regular ovulatory cycles. Medication is effective for most people and surgery is rarely needed.
Dopamine Agonists: Cabergoline vs. Bromocriptine
These medicines mimic dopamine and switch off excess prolactin production.
| Cabergoline | Bromocriptine | |
|---|---|---|
| Dosing | Once or twice a week | Once or twice a day |
| Tolerability | Usually better tolerated | More nausea and dizziness in some |
| Effectiveness | Very high | High |
| Notable point | Convenient; usually first choice | Long safety record in pregnancy |
Managing Side Effects of Prolactin Medication
Possible side effects include nausea, dizziness or light-headedness, headache and tiredness, mostly in the first weeks. Doctors usually start with a low dose and increase it slowly, and taking the tablet with food at bedtime can help. Report any side effects to your doctor.
Restoring Ovulation and Improving Pregnancy Outcomes
The outlook is encouraging: once prolactin is brought back to normal, reproductive function generally returns.
Timeline for Restoring Natural Ovulation
Prolactin often begins to fall within a few weeks of starting treatment. Many women resume periods within about one to two months, and ovulation commonly returns soon after. Most women who have no other fertility factor go on to ovulate and conceive once their levels are controlled.
Managing Prolactin Therapy During Early Pregnancy
The plan depends on the type of tumour, if any:
- Functional hyperprolactinemia or microprolactinoma: medication is usually stopped once pregnancy is confirmed, with clinical monitoring, because the risk of growth in pregnancy is low.
- Macroprolactinoma: management is individual and some women continue treatment under close specialist supervision.
Never stop or change treatment without medical advice.
Hyperprolactinemia vs. Other Hormonal Imbalances
High prolactin is only one of several causes of irregular periods; PCOS and thyroid problems are more common. For the wider picture, see our guide to hormonal imbalance symptoms in women.
Differentiating High Prolactin from PCOS
PCOS also causes irregular cycles, but it is characterised by insulin resistance, raised androgens (acne, excess hair) and polycystic-appearing ovaries. Hyperprolactinemia is primarily a pituitary problem. A hormone panel tells them apart. See PCOS and fertility.
The Overlap with Hypothyroidism
Because an underactive thyroid can raise prolactin, thyroid function (TSH and free T4) should always be checked with prolactin. If hypothyroidism is the cause, treating it may bring prolactin down without a dopamine agonist.
Frequently Asked Questions About Hyperprolactinemia
Can hyperprolactinemia cause permanent infertility?
No. It is generally a reversible barrier to ovulation. Once the cause is found and treated, normal cycles and fertility usually return.
How long does it take to get pregnant after lowering prolactin?
Many women ovulate again within a couple of months of starting treatment. How long conception takes then depends on age and partner fertility.
Is cabergoline safe when trying to conceive?
It is widely used in women trying to conceive. Your specialist will tell you when to stop it after a positive pregnancy test, depending on your situation.
Can high prolactin cause miscarriage?
Untreated hyperprolactinemia can cause a luteal phase defect, which may raise the risk of early miscarriage. Normalising prolactin before conception addresses this.
Can lifestyle changes lower prolactin?
Good sleep and stress management can help prevent minor spikes, but they cannot treat a prolactinoma, a thyroid disorder or a medicine-related cause.
